Behavioral Health Visit - Anxiety/Depression
Patient Name: Kris Markson
The patient initiated this virtual encounter due to the increasing anxiety and depression.
In the past, they have had counseling services by the group that this provider practices. There are not providers available to meet with the patient today. The practice is a Behavioral Integrated Care Practice. Providers available in the practice are Social Workers, Psychiatrists, Primary Care Providers, and Women’s Health Providers.
You are employed through the Behavioral Integrated care practice to triage emergencies and to determine best disposition of prn or emergency visits. The office scheduler asked this care provider to see and screen the patient to determine best course of care.
The patient is very comfortable with telehealth and has had multiple visits virtually over the last several years. The patient actually prefers this type of encounter because they do not have to travel for the visit. They have missed appointments in the past that were in-person.
The learner should identify at the conclusion of the visit what other provider should see the patient. They should ask if the patient would schedule another virtual visit with themselves and the primary care provider. The visit should be collaborative and can be virtual.
Note: Refer to the “TIPS Scale for Telehealth Communication Guidelines” (prior to the visit)
